Four Seasons Home Health Care - FOUR SEASONS HOME CARE AGENCY is seeking compassionate and dedicated Registered Nurses (RNs) to join our team providing in-home care services. As a Registered Nurse in our home care agency, you will be responsible for delivering high-quality patient care, conducting visits, and ensuring accurate documentation in a timely manner. Responsibilities

As a Registered Nurse in our home care agency, you will be responsible for delivering high-quality patient care, conducting visits, and ensuring accurate documentation in a timely manner
Conduct comprehensive patient assessments, develop care plans, and collaborate with physicians to ensure optimal patient outcomes
Perform routine visits to monitor and evaluate patients' conditions, report changes in health status, and update care plans as needed
Supervise Home Health Aide and Personal Care Aide staff in patietns homes
Accurately document patient information, nursing interventions, and progress notes in compliance with agency standards and regulations
Educate patients and their families on health management, treatments, and self-care techniques to promote health and prevent complications
Ensure timely submission of all documentation, reports, and care plans
Participate in team meetings and collaborate with other healthcare professionals to ensure coordinated patient care
Ensure all patient safety protocols and infection control practices are followed

Qualifications

Current, valid Registered Nurse (RN) license in New York
Prior home care experience preferred
Strong clinical skills with an ability to provide direct patient care and develop individualized care plans
Excellent organizational and time management skills to ensure timely documentation and patient visits
Strong written and verbal communication skills
Ability to work independently and manage multiple patient cases
Compassionate, empathetic, and patient-focused demeanor
Ability to work effectively in a team environment
Valid driver's license and reliable transportation for patient visits
