in the community? The Senior Case Manager at Bethesda Project

contribute to the welfare of others every day! The Senior Case
Manager assists individuals with a history of homelessness, chronic
mental illness and substance abuse problems with an array of
supportive services to stabilize their medical and mental health
conditions to prepare them for more independent living. The Senior
Case Manager is responsible for working closely with the Program
Coordinators and Social Service Coordinator to ensure that the
residents are safe and are making healthy choices. Required
Education: Master’s Degree in Social Work Reports to Social Service
Coordinator General Responsibilities Case management of
Shelter-Plus Care residents at scattered Bethesda Project sites;
provide an assessment of each resident’s needs and provide an
individualized care plan for each resident Assist residents to
maintain important relationships and serve as an advocate with
their service providers, legal services and nutritional programs;
assist residents in acquiring needed entitlements Assist residents
during times of crisis, anxiety, apprehension or confusion,
offering appropriate aid and support Coordinate Bethesda Project's
housing application process Develop relationships with referral
agencies to maintain necessary intake lists for each site Share
information about Bethesda Project’s admissions process with
agencies and potential applicants Skill/Educational Requirements
Master’s Degree in Social Work ( R equired ) 1-2 years of
experience preferred. Experience in case management, strong
understanding of homelessness, substance abuse and mental health
issues including a knowledge of the recovery process and drug and
alcohol inpatient and outpatient programs Computer literacy with
proficiency in MS Word, Excel, Outlook Physical Requirements
Ability to travel to multiple Bethesda Project sites Requires
repetitive movement of hands and fingers for typing and/or writing
Frequent standing, sitting, walking and climbing stairs, occasional
stooping, kneeling or crouching Lifting up to 20lbs Ability to
reach with hands and arms and perform audio and visual assessments,
