Job Description Location: Brooklyn, NY Job Type: Part Time - 14 hours per week Facility: Out Patient Facility Responsibilities Provide outpatient nephrology consultations and follow-up care Diagnose and manage chronic kidney disease and related renal conditions Develop individualized, evidence-based treatment plans Collaborate with primary care providers, nursing staff, and care coordination teams Ensure timely referrals and continuity of care Maintain accurate and timely medical documentation Participate in quality improvement initiatives Ensure compliance with federal, state, HRSA, and managed care guidelines Qualifications MD or DO from an accredited medical school Board Certified or Board Eligible in Nephrology Active, unrestricted New York State medical license Active DEA registration Hospital privileges (or ability to obtain) Commitment to patient-centered, culturally responsive care Strong communication and teamwork skills Compensation & Benefits 5% 401(k) employer contribution Standardized PTO structure Malpractice coverage Safe and Sick Leave
