Our department, the Department of Population Health, aims to improve Brooklyn's community health through system transformation and coordination across various health and social services organizations. As a Central Services Organization (CSO), we support primary care, care management, and community partners, managing entities like Community Care of Brooklyn IPA, Brooklyn Health Home, LLC, and Brooklyn Communities Collaborative, Inc. We also serve as an academic hub for research, program evaluation, and dissemination of knowledge about integrated delivery systems.
Responsibilities

• Support care management referrals, member transitions, and network performance as part of the navigation team.
  
• Complete assessments via telephone with community members and providers to facilitate referrals to Brooklyn Health Home and Community-Based Organizations for enrollment in care management, behavioral health, or social services. Support and monitor a small caseload to ensure connection to services.
  
• Collaborate with all levels of CSO staff and communicate directly with patients, health members, care providers, community organizations, and managed care entities.
  
• Conduct behavioral health/social service assessments for referrals.
  
• Assist clients with complex needs in applying for programs like HRA, SNAP, Medicaid, and Medicare.
  
• Manage assigned cases, source new social care resources, and ensure service connection and documentation.
  
• Follow up with care management and supervisory staff regarding referrals.
  
• Perform chart reviews for quality assurance as needed.
  
• Escalate issues to the Program Manager as necessary.
  
• Support Health Home referral requests and coordinate with stakeholders.
  
  Qualifications
  

• 
  
• Bachelor's degree.
  
• At least two years of experience in healthcare, social services, or community health.
  
• Experience assisting with applications like HRA, 2010e, SNAP, and navigating Medicaid/Medicare.
  
• Strong communication skills; bilingual abilities in Spanish, Russian, Mandarin, or Cantonese are preferred.
  
• Ability to work in a fast-paced, collaborative environment.
  
• Excellent organizational and time management skills, with proficiency in database programs.

  Pay Range
  USD $50,000.00 - USD $55,000.00 per year
