Job Summary We are seeking a friendly and compassionate Primary Care Provider who is willing to relocate to Vermont to join our team. As a Primary Care Provider, you will play a crucial role in providing comprehensive care to patients in long-term care facilities. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals. Requirements Provide comprehensive healthcare services and coordinate patient care Work closely with a team of healthcare professionals Conduct thorough patient assessments, including medical history reviews and physical examinations Order and interpret diagnostic tests Develop accurate diagnoses and treatment plans Prescribe medication and administer treatment Collaborate with the interdisciplinary team for seamless coordination of care Manage chronic conditions and monitor progress Make appropriate adjustments to care plans Commitment to person-centered care Create a safe and nurturing environment for elderly residents in the nursing home Requirements: Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license State-specific license in good standing in Vermont Minimum 6 months of experience in long-term care or a skilled nursing facility Strong communication, clinical, and critical thinking skills Passion for elder care and person-centered healthcare Ability to relocate to Vermont Benefits Competitive salary: $150,000 – $200,000/year Health Dental Vision 401K Company-Sponsored Life Insurance CME Reimbursement and PTO Paid Time Off Malpractice Insurance $2,000 reimbursement for relocation expenses $10,000 relocation bonus after a successful 90 days of employment Equal Opportunity Employer Infinite Medical P.C is an equal-opportunity employer.
