Job Description Job Description Clinical Manager Responsible to:
Director of Patient Services Description: Responsible for assessing
patient care needs and initial planning for patients referred to
home health care services. Provides guidance and oversight to
clinical field staff to ensure regulatory compliance, optimal
quality of care and case management and timely, accurate
documentation. QUALIFICATIONS 1. License to practice as a
registered nurse in the state, BSN preferred 2. Minimum of three
years nursing experience required 3. Two years of home health
nursing preferred 4. Knowledge of OASIS and clinical documentation
standards 5. Effective verbal and written communication skills 6.
Excellent problem-solving skills 7. Ability to organize multiple
responsibilities and set priorities 8. Excellent skills in customer
relations DEGREE OF TRAVEL Moderate degree of travel to assess
patients in institutional settings and/or home environment
