Registered Nurse (RN) | Home Health Location: Philadelphia, PA Agency: Revolution Healthcare Services Pay: $2,756 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Opportunity – Home Health Registered Nurse – Pennsylvania Start Date: February 2, 2026 Location: Philadelphia, Pennsylvania Compensation: Details provided to qualified applicants Schedule: 6 patients per day with weekly productivity of 30; possible weekend coverage; no call required Highlights: Provide holistic care to patients with multiple co-morbidities and advanced chronic illnesses in their homes Complete initial visits, revisits, start of care/resumption of care, recertifications, and discharges Utilize Homecare Home Base EMR for documentation and care coordination Mileage reimbursement provided at competitive federal rate Manageable caseload supporting work-life balance with predictable patient volume Serve designated zip codes within the service territory Requirements: Active Pennsylvania RN license BLS certification (American Heart Association) Minimum 1 years of recent Home Health experience required Case management experience required Proficiency with Homecare Home Base EMR system required Previous travel experience highly preferred IV and venipuncture experience highly preferred Valid driver's license with clear driving record and current auto insurance (proof required within 48 hours of offer acceptance) Why It's a Great Opportunity: This position offers the rewarding experience of providing compassionate, patient-centered care in a home setting where you can truly make a difference in patients' and families' lives.
