Must-Have: Proficiency in care coordination and documentation
Description: Accountable for preparing, organizing, and assisting
Case Managers. Coordinates and maintains all office management
functions. Independently manages patient discharge functions such
as transportation and nursing home returns. Intervenes with
insurance companies on utilization review activity. Conducts
monthly calls to all members assigned and ensures compliance with
verification protocols. Utilizes assessment tools when
communicating with members. Employs critical thinking and judgment
when dealing with unplanned issues. Escalates clinical issues to
designated care managers when necessary. Accurately and timely
documents member call interactions in the care management system.
Ensures members have appointments with their providers. Fulfills
basic care coordination tasks for members, such as scheduling
appointments and addressing medication issues. Conducts supportive
activities as assigned. Problem-solves member issues independently
and resourcefully. Performs non-clinical activities for members of
varying ages and backgrounds. Engages members and caregivers in a
collaborative relationship. Empowers members to manage their
physical, psychosocial, and environmental health. Promotes
effective communication both internally and externally. Maintains
accurate documentation in compliance with regulatory and
accreditation requirements. Protects the confidentiality of member
information and adheres to all privacy policies. Collects and
prepares data, statistics, proposals, and reports relevant to
responsibilities. Provides care coordination that aims to reduce
avoidable utilization and improve member satisfaction. 5 Benefits
