Traveler is seeking an experienced RN for a Home Health assignment
in New Jersey requiring strong recent home health expertise and
OASIS experience. Responsibilities - Home health care setting
covering patients recently discharged from hospitals and nursing
facilities - Patient population includes wound care, wound vacs,
pleurx drains, ostomies, heart failure, diabetic, oncology,
post-surgical, and orthopedic cases - Productivity expectations are
6 visits per day or equivalent (e.g., admissions count as 2) - Day
shift schedule (5x8 hours) from 8:30 to 17:00 - Required to work
every 5th weekend and every other holiday - No mileage
reimbursement provided - Use of OASIS for assessments and EPIC EMR
for documentation - Travelers may be required to float within 5
hospitals in the service area, depending on staffing needs -
Primary responsibility includes providing skilled nursing care and
completing home health assessments - Role involves wound care, IV
therapy, and monitoring chronic conditions - Applicants must be
able to work independently and manage a varied home health caseload
- Orientation modules will be completed as part of the onboarding
process - Must work individually; travel pairs are not permitted -
RTO must be declared at time of submission, with preference for
requests under four days and no time off during the first week -
Dress code is professional attire suitable for home visits
Requirements - Active NJ or Compact RN license required and must be
in hand at the time of consideration - Minimum of 2 years recent
home health RN experience within the last 12 months is required -
Med Surg nursing skills required - OASIS and EPIC experience
required - Current BLS certification required - Valid driver's
license and reliable transportation required - Proof of COVID and
Flu vaccination required at the time of consideration - Must pass a
dysrhythmia test during orientation (two attempts permitted)
