are currently recruiting for a compassionate Home Health Palliative
Care Nurse Practitioner (NP) to reliably serve patients in
Gloucester, Salem, and Cumberland County, NJ. This is a full-time,
salaried position. What you'll do: As a BAYADA at Inspira Health
Advanced Practice Registered Nurse/Nurse Practitioner: you will use
your clinical skills and compassionate heart to treat patients
where they want to be the most, home. A member of the
multi-disciplinary clinical team to provide initial and ongoing
clinical assessment for clients with advanced and or progressive
medical conditions. Key objectives of this role include:
2. Master's Degree with Advanced Practice Registered Nurse (APRN).
Current license in good standing.
3. American Nurses Credentialing Center (ANCC) or American
Association of Nurse Practitioners (AANP) Board Certification
required.
4. Prefer a minimum of one (1) year of recent, verifiable,
experience as an Advanced Practice Nurse in palliative care, home
health and/or hospice care.
5. Prefer Palliative Care Certification within one (1) year of
employment.
6. Autonomously manage competing priorities in a fast-paced
environment that includes travel to multiple sites and geographical
communities.
7. Possess strong communication and interpersonal skills including
the ability to cultivate a positive and energetic team
atmosphere.
8. Have proficiency in basic software applications required to
perform job functions.
9. Achieve a passing score on written examinations.
10. Completed criminal background and federal/state program
exclusions checks.
11. Ability to read, write and effectively communicate in
English.CLINICAL RESPONSIBILITIES :
1. Demonstrate and communicate the core values of BAYADA and the
BAYADA WAY.
2. Works under collaborative agreement with an Interdisciplinary
physician and as required by state.
3. Understands and complies with federal, state and local
regulations that govern the practice of APRNs.
4. Evaluates and develops care plans in accordance with accepted
standards of clinical practice and adjusts plans as needed.
5. Recommends, performs and interprets laboratory and radiology
tests within scope of professional practice.
6. Recommends medications including controlled substances to the
extent elegated and licensed.
7. Provides recommendations for treatments and durable medical
equipment as indicated. 8. Performs other therapeutic or corrective
measures as indicated, including referral to urgent care, pain
management clinics, grief counseling or behavioral health
services.
9. Provides clinical care and education to manage the medical and
social needs of palliative care clients in the community setting
through advance care planning, pain management, symptom management,
goals of care and timely referral to appropriate resources.
10. Prepares and maintains accurate electronic medical records and
other documents to support sound medical practice and
reimbursement. Complies with applicable laws, regulations and
clinical practice standards.
11. Facilitates care coordination and transition between hospitals
and physicians, referral source, the palliative care team, home
health care team and hospice team.
12.Completes Face to Face as requested for hospice clients in
accordance with all regulations.
13. Collaborates with team members to provide education and foster
effective partnerships with all community-based agencies.
14. Collaborates with the designated hospital partner in
implementing strategies and risk stratification for home health and
palliative care clients in the community setting to prevent
avoidable hospitalizations.
15. Attends and provides expert palliative care consultation for
complex pain and symptom management during team and family
conferences in collaboration with the clinical team.
16. Provides education and counseling for clients and families
about chronic disease process, symptom management, pharmacological
and non-pharmacological options for care and treatment.
17. Provides education to team members regarding chronic disease
process, symptom management, pharmacological and
non-pharmacological options for care and treatment.
18. Collaborates and coordinates with the Client Services Manager
and hospice team in transitions to hospice as appropriate.
19. Consults with referring physician or designees as needed,
informs primary physician of services provided and collaborates
with other physicians as needed.
20. Participates in program growth and development as it relates to
high risk medically complex patients.
21. Adheres to BAYADA Home Health Care's policies of
confidentiality regarding patients, families and
staff.ADMINISTRATIVE RESPONSIBILITIES:
1. Reports any changes to Division Director.
2. Prepares and submits clinical notes and assessment/evaluation in
a timely fashion.
3. Is reliable and punctual.
4. Demonstrates an appropriate attitude.
5. Has a positive rapport with clients and families, as evidenced
by client satisfaction.
6. Meets deadlines for ongoing in-service training and health
screening requirements.
7. Follows BAYADA policies and procedures.
8. Other responsibilities, as required:TRAINING AND
CERTIFICATIONS:Within the first three (3) months of employment,
1. Completion of The BAYADA Way In-service.
2. Completion of White Cap Orientation and Training at Director's
discretion.
3. Completion of general company Welcome to New Employees
Training.
4. Basic Cardiac Life Support (BCLS) Certification
5. Complete all necessary competency requirements related to
