AristaCare At Cherry Hill - Social Worker
AristaCare at Cherry Hills is currently looking for Social Worker. In this role, -you will assist with planning, developing, organizing, implementing, and evaluating, the social service programs of this facility. - -compensation and enforcing company policies and practices.
- - - - - -This position is full-time.
- - - - - - -Responsibilities include but not limited to:
- - - - - - - - - -Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
- - - - - - - - - -Participate in discharge planning, development and implementation of social care plans and resident assessments.
- - - - - - - - - -Involve the resident/family in planning social service programs when possible.
- - - - - - - - - -Assist in arranging transportation to other facilities when necessary.
- - - - - - - - - -Refer resident/families to appropriate social service agencies when the facility does not prove the services or needs of the resident.

Requirements

Associate Degree required.
2- 3 years' experience in a Skill Nursing or Assisted Living required
Excellent interpersonal and conflict resolutions skills.
Dementia Experience
